Itemized billing statement with ICD-10 diagnosis codes, CPT procedure codes, a letter of medical necessity from the treating clinician, and proof of payment.
You file an out-of-network claim
Request your insurer's OON claim form before discharge and submit the packet. Reimbursement, when approved, is a percentage of the allowed amount after your OON deductible.
